Candle Lighting Ceremony & Memorial Book

During the candle lighting ceremony, the children's names will be read along with a slide presentation displaying the children's photographs and/or names. Woody Wolfe will also perform several songs. Candles, glow sticks and complimentary parking will be provided.

• If you plan to attend, you may bring up to 6 guests.
• This event is standing room only. Chairs only provided to those who cannot stand.

You will receive a confirmation email when your R.S.V.P. is received. You do not need to print ticket(s).

Memorial book and slide presentation

Whether or not you can attend, you are invited to include a photo, written tribute and/or your child's name in the memorial book.

To be included in the memorial book, your photo, written tribute and/or child's name must be received by Wednesday, Oct. 1.

• We sincerely regret that we cannot include any photo, tribute and/or name received after Oct. 1 in the memorial book.
• Any photo and/or name received after Oct. 1 but by Monday, Dec. 8 will be included in slide presentation only.

Submitting tribute and/or photo

• Email one written tribute and/or one digital photo (JPEG format) to evenstar@email.chop.edu
• Include child's name in subject line of email
• Proofread tribute carefully and include child's name, date of birth, and date of death
• You will receive a confirmation email when your tribute and/or photo are received
